DISCOUNT PLAN
True Dental Plan is an innovative dental plan that provides members with significant savings on dental services in participating dental offices. It is an easily
accessible plan offering a complete package of reduced fees for most of your dental needs. True Dental Plan is not supplemental dental insurance. It is a discount
dental plan in which participating dental providers have agreed to provide plan members certain dental services at reduced fees. As a member of True Dental
Plan, you are entitled to a predetermined reduced cost of services that True Dental Plan has pre-negotiated with the Dentists on your behalf. A complete List of
Services and Fees is included in this package for your information and can also be downloaded from our website at: www.truedentalplan.com. Fees are subject to change without notice. True Dental Plan Member is responsible for paying the fees listed on the List of Services and Fees at the time treatment is received. There are no waiting periods and no yearly maximums. The yearly membership premium is due with submission of this application.
PLAN ELIGIBILITY
Full Time or Part time Registered ASU Students are eligible to enroll in True Dental Plan. Current Student ID is required for enrollment.
EFFECTIVE DATE
Enrollment is based on the date of sign up. Effective dates run for 1 year (365 days) from date of enrollment. Membership to True Dental is effective immediately upon processing of the member application and payment. Any questions regarding eligibility can be made by calling 480-967-3493.
CANCELLATION POLICY
True Dental Plan can be cancelled within 30 days of the purchase date. If the plan is cancelled within 30 days, a $15 processing charge will be deducted from the
refund and the patient will be billed the usual and customary charges for all procedures performed prior to the cancellation. Please see the Brochure or the UCR
fees. Refunds will be processed promptly and may take 2-4 weeks.
Notice to Plan Members with Current Dental Insurance
True Dental Plan cannot be used in combination with a current dental insurance policy for procedures covered by the insurance policy. True Dental Plan may be
used together with a dental insurance policy in the following ways: 1. after a patient hasexceeded coverage on a dental insurance policy, 2. if certain services are
not a covered benefit under a dental insurance policy. A dental Insurance policy will always remain the primary insurer for covered services provided by our
participating providers.
Additional Limitations and Exclusions
Service provided by non-participating dental providers are not a covered benefit. Loss or theft of any dental appliance is not covered. Out of area emergencie
are not a covered benefit. Only procedures performed at contracted offices are covered under this plan. Any dental procedure not listed on the plan fee schedule
will be offered at 10% discount of the provider’s usual fees, including implant restorations, porcelain veneers, zoom whitening, Invisalign orthodontics.
(2) No Charge Dental Emergency office visits are a covered benefit only during each provider’s normal business hours. Weekend/on call visits will be billed at
usual and customary after hour fees. No Coupons or Referral Discounts can be used in conjunction with True Dental Plan.
